When selecting a Personal Assistant or Provider Agency, it is important that you make your decision based on who you feel would best meet your needs. Below are some questions to ask the provider agency that can help you decide whether or not they are the best choice for you to work with.

bullet What type of services do you provide?
bullet Do you have emergency services available after hours?
bullet If so, how do I access them?
bullet How will I be involved in this process?
bullet How much say do I have in the services that I will receive?
bullet Do you have any brochures or a website which may better explain your program and its associated services?
bullet Will there be a written plan of care?
bullet If so, how often is it updated and how will I be involved in its development?
bullet Are there any limitations to the type of service that can be provided and/or when it could be provided?
bullet Can I meet a personal care attendant before they begin working with me?
bullet What type of training do your personal care attendants receive before they begin working?
bullet Is there any additional screening? Who would I call if I had a problem with my personal care attendant or if they don’t show up for work when scheduled?
bullet What is the procedure if I have a complaint?
bullet Who supervises the personal care attendants? How often will someone from your agency be coming out to check on the progress of our working relationship?
bullet Can I see a copy of the contract I may be required to sign?
